The Colissimo CPSC annex, field by field
Two forms exist: one for products registered with CPSC, one for everything else.

For a registered product the annex carries three identifiers - Product ID, Certifier ID, Version ID; without registration, seven elements are required for each shipment concerned.

One form per parcel, before the label
The annex is not a document you keep: it is attached to each shipment in the postage tool, at the moment you prepare it. Which means it is part of your shipping routine rather than part of your paperwork, and that is the mental shift most sellers have to make.

There are two versions of the annex, and the difference is whether your products are recorded with CPSC. The short one lists three identifiers per item; the long one asks for the seven data elements, per item, on every parcel.

Choose the form once and keep to it. Mixing them - registered identifiers on the long form, or half the seven elements on the short one - produces a document that reads as incomplete rather than as a different choice, and that is the state a shipment gets held in.

The tracking number field stays empty. It looks like an omission, and sellers fill it with the order number or a previous parcel's tracking, but the parcel has no number yet at the moment the annex is prepared.

None of these three is a judgement call, which is the good news: they are transcription, and transcription can be turned into a habit. Prepare the annex once for a product, keep it beside the listing, and the per-parcel work is a description and a click.

Frequently asked questions
Which of the two forms do I use?
The unregistered-goods form itself directs you: if the product is registered you complete one set of lines, if not, the other.
Several products in one parcel?
The annex has one line per item, and Colissimo asks for one form per parcel, so the items of a parcel travel on the same annex.
What if my product needs no certificate?
The annex then carries a Disclaim: a disclaimer code A or B, an intended use code and its description.
