Every field below maps to a paragraph of 16 CFR 1110.11(a), as amended on 8 July 2026. Filling it in is free. (a)(1) Product

16 CFR 1110.13(a) requires a certificate to describe only one product, and a unique identifier has been mandatory since 8 July 2026.

(a)(2) Rules certified to

Each applicable rule must be stated separately. These are the same data elements that get eFiled into ACE at entry. Which rules apply to your product is your determination, not ours.

(a)(3) Finished product certifier

For goods manufactured outside the United States this is the importer, under 16 CFR 1110.7 — not the overseas factory. An email address is required here.

(a)(4) Individual maintaining records

A position title is acceptable provided the position is always staffed and responsive to CPSC. These are the records 16 CFR 1110.17 requires to be kept for five years.

(a)(5) Date and place of manufacture

For a manufacturing run spanning several days, give the initial date.

(a)(6) Date and place of testing

The date required is the most recent one. If you claim a statutory or regulatory testing exclusion, 16 CFR 1110.11(c) puts the identification of that exclusion in place of these details for the rule concerned.

(a)(7) Attestation

The rule prescribes this wording. It will appear on the certificate exactly as written below.

I hereby certify that the finished product(s) covered by this certificate comply with the rules, bans, standards, and regulations stated herein, and that the information in this certificate is true and accurate to the best of my knowledge, information, and belief. I understand and acknowledge that it is a United States federal crime to knowingly and willfully make any materially false, fictitious, or fraudulent statement, representation, or omission on this certificate.
